Investigation Summary
Employee #1 was using a powered horizontal conveyor to weigh and band 12,000 pound brass coils. The coils are moved by paddles on an I-beam under the conveyor rollers. The machine was either running or accidentally started while Employee #1 was reaching between two coils. His head and right shoulder were crushed and Employee #1 was found dead several minutes later.
