Employee killed when struck by falling beam

On December 10, 1991, Employee #1 was welding a beam that was about 63 ft long by 4 ft high by 21 in. wide and sitting on 12 in. steel channels, but not secured to them. The beam had a 2 ft 11 in. extension welded to it. An Eagle forklift struck the extension, causing it to tip over onto the employee. He was killed.
