Employee killed when caught by moving drives of equipment

Employee #1, the #12 broke beater operator at a paper mill, was found dead behind the barrier panels of the #12 after-dryer equipment. Apparently, he became caught by the moving drives and sustained a head fracture. The barrier panels were only fastened at the top and easily permitted entry to the hazardous moving drives. In addition, two unguarded openings were on each side of the barrier. One of Employee #1's duties is to clean up. He was found with an air hose lying across his body.
