Investigation Summary
On October 27, 1995, Employee #1 was on the north side and near the top of a vertical form. He was trying to connect a work platform to the form. Employee #1, who was wearing a positioning belt and lanyard, was holding his body close to the face of the form while the crane operator held the work platform over the employee's head. Employee #1 leaned back in his belt and fell. His head was fractured and he was killed.
