Employee is crushed and killed by forklift

At approximately 11:00 a.m. on March 3, 2004, an employee drove the forklift about 200 yards on a gravel road from a warehouse toward an area where coworkers were stacking scaffold frames. From the forklift's tire tracks, it appeared that the forklift was driven off the road's surface onto some debris and soft ground. When the employee turned the forklift back toward the gravel road, it looked like the tires went into the soft dirt and the forklift rolled onto its side. The employee was pinned between the gravel road and the forklift's canopy. Rescuers stated the canopy was just above the employee's hip. The employee was taken to a hospital and died. There were no witnesses to the accident.
