Investigation Summary
At approximately 8:24 a.m. on November 18, 1991, Employee #1 was operating a John Deere crawler loader, model #CH0250X096, serial #0290104059. He climbed from the loader's right side while it was moving, ran around in front of the loader, and lay down about 3 to 5 feet ahead of the moving loader. The loader ran over and killed him. When a coworker climbed up on the loader to shut it off, he found a piece of wooden folding ruler wedged against the gear shift. The Wilmette Police Department classified the accident as a suicide, based on obtained employee statements.
