Investigation Summary
A CONSTRUCTION CREW WAS HOISTING AND POSITIONING A PREFABRICATED RISER. ONE EMPLOYEE WAS STANDING ON THE RISER OPERATING A CHAIN HOIST. ANOTHER EMPLOYEE (EMPLOYEE #1) WAS STANDING UNDER THE RISER DIRECTING ITS PLACEMENT. THE SUPPORTING BEAM, COMPOSED OF ALUMINUM AND WOOD, SUDDENLY ROLLED, BENT AND BROKE. THIS ALLOWED THE HOIST RIGGING TO COME LOOSE FROM THE BEAM, WHICH IN TURN CAUSED THE RISER TO FALL. EMPLOYEE #1 WAS STRUCK AND KILLED. THE EMPLOYEE STANDING ON THE RISER (EMPLOYEE #2) FELL WITH THE RISER BUT SUSTAINED ONLY MINOR INJURIES.
