Investigation Summary
Employee #1 was driving a truck that was positioned behind a log truck being unloaded at the Mead Coated Board, Inc., plant. A mill worker was using a backhoe with a boom and paddle to push short logs into the flume. A log apparently became jammed underneath a tire on Employee #1's truck, and he tried to remove it. The coworker did not see him, and Employee #1 was pushed by the backhoe into the log flume. He was killed.
