Employee suffocates in large amounts of sawdust

Employee #1 was working inside a fuel house in a sawmill, pitching shavings to a center chain that carried them to the boiler. He inadvertently stepped to the center of the room and struck the chain, which dumped large mountains of sawdust on him. He was engulfed by the shavings, suffocated, and died.
