Investigation Summary

Investigation Nr: 170577746
Event: 07/09/1995
Employee dies after legs are crushed by steel rolls

Employee #1 was using an overhead crane to unload rolls of coiled steel from a flatbed tractor trailer. He was storing the rolls of steel in a barricaded storage area. Employee #1 positioned the crane between two stored rolls, lowered another roll, and placed it on top of the two lower stored rolls. The weight of the top coil caused the two lower coils to spread apart and crush Employee #1's leg between two of the rolls. He was transported to the hospital, where he died after refusing to receive a blood transfusion because of his religious beliefs.
