Investigation Summary
Employee #1, a shipfitter, was welding a transverse girder to a barge deck. He rigged a shopmade hook, called a packman, to an angle longitudinal girder to bring the deck and the transverse girder together. The packman was pulled into position by a 20 ton crane, which inadvertently bent the angle longitudinal girder. The packman fell off the girder and struck Employee #1. He died from head, face, and neck injuries.
