Investigation Summary
A crew of three employees were pulling oil well casing from a well for salvage. They had just removed the "shot can" laden with solidified nitroglycerin from the well after the can failed to reach the required depth. The crew attempted a couple of times to lower the shot can to the 4,300 ft depth, but was unable to lower the can below the 1,640 ft level because of an unknown obstruction. The shot can was lifted from the well and as Employee #1 tried to disarm it, the explosive charge detonated, injuring the three employees. Employee #1 died several days later at a Wichita hospital. Employee #2 suffered the amputation of three fingers on one hand and was hospitalized. Employee #3 was treated for lacerations and burns on one hand and released.
