Investigation Summary
At approximately 9:00 a.m. on August 13, 1991, Employee #1 was working as a faller on a hillside approximately 150 ft above a logging road where he was opening up a cutting strip. He attempted to fell the twelfth consecutive tree (a small 19 in. diameter hemlock) so that it would fall slightly downhill and to the left of a large 36 in. diameter hemlock that was located 36 ft away. The small tree lodged in the large hemlock, resulting in a hang-up. The employee chose to clear the hang-up by felling the large hemlock. He cleared out his escape path to the right and then felled the large hemlock across a small drainage and slightly to the right. The falling hemlock carried the small lodged tree over the escape path so that it fell across the escape path. The small tree then deflected downhill toward the large stump, crushing and killing Employee #1, who had apparently stepped in a small hole and tripped.
