EMPLOYEE'S THUMB FRACTURED DUE TO GUARD MALFUNCTION

On March 6, 1990 Employee #1 was operating a small mechanical power press which was activated by a foot pedal and guarded with a radar proximity guard. The radar proximity guard was supposed to prevent machine operation if anything was close to the point of operation. Employee #1 attempted to remove a piece of work from the press. He stated that his foot must have accidentally touched the pedal. The press was activated because of the apparent malfunction of the radar proximity guard. Employee #1's right thumb was fractured.
