EMPLOYEE KILLED WHEN MULTIPIECE RIM WHEEL EXPLODED

EMPLOYEE #1 WAS INFLATING A MULTIPIECE RIM WHEEL OUTSIDE OF A PROTECTIVE CAGE. THE TIRE EXPLODED, AND THE RIM WHEEL STRUCK THE EMPLOYEE ON THE FOREHEAD, CAUSING A FATAL INJURY.
