Investigation Summary

Investigation Nr: 170683866
Event: 05/30/1992
Employee hospitalized when hand caught in rollers

Employee #1 was operating a paper bag forming machine when he saw that an edge of the paper had folded over. Employee #1 attempted to correct this condition without stopping the machine. He stood in an awkward and unbalanced position on a raised portion of the floor that was supporting the press and placed his hand near the roller. As he tried to straighten the paper going into the rollers, his fingers were caught in the rollers and he was pulled into the machine. A worker assisting him was at the shut down switch and saved him from further injury by stopping the press. Employee #1 was hospitalized. It was a violation of company policy to try to make the correction without stopping the machine.
