Investigation Summary

Investigation Nr: 201560018
Event: 12/10/1992
Employee's fingers lacerated in table saw

At 6:00 a.m. on December 10, 1992, Employee #1, of Alms, Inc., reported for work. He was directed to use a slider table saw to cut and rip wooden sheets. Employee #1 did not secure the fence of the saw correctly. It was secured to the rear of the saw table and did not provide the proper security for the wood as it passed through the blade. As he proceeded to cut the wood, it kicked back in the blade area. When Employee #1 reached across the moving blade to clear the problem, his left hand struck the saw blade. He lacerated his little finger, which was later amputated by a doctor at the hospital. His left ring finger, thumb, and hand also sustained cuts. Employee #1 had been employed since November 23, 1992 and had received three days of training from a coworker on how to use the saw.
