Investigation Summary

Investigation Nr: 691451
Event: 11/04/1987
Employee killed when water tanker rolls over

Employee #1 was operating a Caterpillar 641 water tanker that was about half full. As he made a turn from one access road to another, the left rear wheel rode up on an approximately 2 ft high beam. This caused the water inside the tanker to surge forward and to the right. The combination of this and the slope of the road caused the tanker to roll over 1 1/4 times, ejecting Employee #1. He was killed when crushed under the windshield and operating area.
