Employee injured in fall from scaffold

Employee #1 was carrying mud west to east on the second level of a scaffold. He made 5 to 6 trips before stepping on the planking and displacing it. He fell 13 ft to the ground along with the planks, which were 2 in. by 10 in. by 16 ft. A cross brace on the first level broke his fall. Five planks filled the scaffold platform.
