Employee killed when struck by steel bars

Employee #1 and a coworker were attempting to hook up and move three bundles of 12 ft steel bars with an overhead crane. The bundles were next to the floor and Employee #1 was on her knees at the end of the bundles hooking a web sling under the stock. Meanwhile, her coworker was walking on top of the bundles removing dunnage (sections of 4 by 4 in. wood). One piece of the dunnage appeared to be wedged between a 19 in. concrete platform and a stack of steel bar bundles that was next to the three bundles of steel to be moved. When the coworker kicked the piece of dunnage loose so that he could remove it, the bundles shifted. He called out a warning to Employee #1 and jumped clear of the area. Two bundles, each weighing 4,000 lb, fell on her. Employee #1 was crushed and killed.
