Employee's thumb lacerated by press

Employee #1 was freeing a stuck part from an eyelet press, model WT, manufactured by USM Corporation, company #010221, serial #2070, when the bolts under the press that held the air cylinders came loose. The air pressure on the rod holding the dog (which stops the press stroke at 360 degrees) was released and the press descended. Employee #1's thumb was lacerated.
