Investigation Summary

Investigation Nr: 795294
Event: 01/05/1991
Employee killed as rim wheel explodes

At 11:30 a.m. on January 7, 1991, Employee #1 was working at his part-time job driving a 1977 GMC flatbed truck for Zerniak Auto Parts. While returning from Corfu, NY with a load of 11,000 lb of steel, he noticed that his left rear tire was low on inflation. He was near his full-time employer, the Town of Sweden Highway Department, so he pulled in to fill up the tire with air. He parked the truck parallel to the building, facing east in front of the third bay door. While he was trying to fill the two-pieced rim wheel with proper equipment, the rim wheel exploded, causing massive chest, neck, and head injuries. Employee #1 was pronounced dead at 12:09 p.m. at Lakeside Memorial Hospital, Brockport, NY.
