Accident Investigation Summary
Summary Nr: 75218.015Event: 03/23/2015Employee Crushed By A 2,000 Lbs. Paper Bale That Had Fallen
At 1:25 p.m. on March 23, 2015, an employee and two other employees were working outside the warehouse, in the loading area; they were cleaning unwanted debris from the paper bales before they were to be loaded to a truck for transport. Th e recycling bales were stacked up 3 tiers high, approximately 14 feet high aroun d the property. Two employees were working on the south of a bale and the employ ee was working on the north side. A 2,000-pound bale fell fatally injuring the e mployee. When Fire Department arrived on scene, it took 10 people to remove the bale off of the employee. The employee was killed from multiple blunt force inj uries to head and entire body.
