EMPLOYEE KILLED WHEN HEAD IS CAUGHT BETWEEN BEAM AND BAR

BECAUSE OF THE LIMITED SPACE AND IN HIS EFFORT TO SEE, EMPLOYEE #1 POSITIONED A STEEL DRILL BAR SO THAT HIS HEAD WAS BETWEEN THE BAR AND THE HANG UP. WHEN THE HANG UP RELEASED, IT DROVE THE BAR DOWN INTO THE CONVEYOR BELT. THE BELT WAS MOVING IN THE OPPOSITE DIRECTION OF THE BAR. THE BELT GRABBED THE BAR AND FORCED THE HANDLE END ONTO EMPLOYEE #1'S HEAD. HE WAS KILLED WHEN HIS HEAD BECAME CAUGHT BETWEEN THE BAR HANDLE AND AN I BEAM.
