EMPLOYEE #1 WAS ATTEMPTING TO REPAIR A LEAKING SPRING BRAKE ON A SEMI- TRAILER, RATHER THAN REPLACE IT. ONE OF THE SPRING RETAINER BOLTS TWISTED OFF, RELEASING THE SPRING. THE SPRING STRUCK EMPLOYEE #1 ON THE HEAD, FRACTURING HIS SKULL AND KILLING HIM.
