Employee injured when grinding wheel shatters

Employee #1 was preparing to true up the face of a grinding wheel that was mounted on a Themacs tool post grinder. As the guard on the grinder was turned on, the wheel shattered and struck him in the right upper chest. He sustained cuts and lacerations and was hospitalized. Subsequent investigation revealed that the pulleys on the motor and spindle shafts were reversed, causing the wheel to travel more than four times the rated wheel speed of 3,600 rpms. The no-load motor speed is 7,500 rpm; therefore, the estimated grinding wheel speed with a pulley reversal of 2 to 1 was up to 15,000 rpms.
