Investigation Summary

Investigation Nr: 892125
Event: 09/10/1991
Employee killed when run over by truck

Employee #1 and coworkers had been working all day laying pipeline in the vicinity of Owings Mills, MD. At the end of the day, three vehicles traveled back to the company yard. The lead vehicle was a double welding rig with just the driver. This was followed by a pickup truck with three employees inside the cab and a third truck with just the driver. The vehicles came to a stop sign where they would turn right and then angle over into the left turn lane. The welding rig turned right and started to pick up speed, attempting to get over into the left turn lane. The pickup pulled up to the stop sign, with the third truck right behind it. Employee #1, who was sitting next to the passenger's door in the pickup truck, undid his seatbelt, opened the door, and started running after the welding rig. He apparently wanted to ride the rest of the way back to the yard with the driver, who was a close friend. Employee #1 ran up to the middle of the welding rig's right side and reached for the oxygen acetylene cables on the rear of the truck. As he grabbed for the cables, he stumbled and fell and the right rear tires of the welding rig ran over his chest and head. He was pronounced dead at the scene. The incident was caused by employee error and no safety violations were cited.
