Employee's fingers crushed by cabinet in fall from ladder

Employee #1 was removing equipment from a location using a fiberglass stepladder. He unbolted one end of a 4-foot long ceiling cabinet after he had removed the grease reels from the cabinet. He then had his helper hold the unbolted end while he unbolted the other end with a wrench. As the nut came loose, the sudden weight of the cabinet knocked Employee #1 from the ladder and the cabinet landed on his hand on the concrete floor, causing the amputation of 3 fingers. The fall was about 6 feet.
