Employee killed in fall from lift truck

On April 21, 1992, Employee #1 was attempting to restack cartons in an elevated rack system. During the operation Employee #1 had been raised on the forks of a lift truck. Before Employee #1 was raised, a pallet had been placed on the forks. After being raised, Employee #1 placed two cartons on the pallet behind him, and then began restacking the other cartons that were in front of him. The cartons shifted, causing Employee #1 to lose his balance and fall over the cartons he had placed behind him. Employee #1 struck his head on the concrete floor, sustained severe head injuries, and died.
