Employee injured when pinned underneath trailer

At 3:15 p.m. on January 7, 1994, Employee #1 and a coworker, both employees of Top City, were installing axles on a 16 foot Karavan utility trailer. The trailer was suspended about 7 feet above the ground by chains looped over the ends of the 10 foot forks of a Towmotor D-41 forklift. While Employee #1 was under the east side and the coworker was under the west side of the trailer tightening bolts, the chains slipped off the end of the forks, causing the trailer to fall. Employee #1 was pinned to an 8 foot utility trailer that was 36 inches east of the 16 foot trailer prior to the accident. Employee #1 was taken to PGRMC where he was admitted and treated for internal injuries.
