Investigation Summary

Investigation Nr: 170070288
Event: 10/27/1992
Employee injures toes on drilling rig

Employee #1 was installing a residential water well to a depth of 500 ft. At approximately the 300 ft level, mechanical problems developed with the drilling rig. A broken hydraulic line at the head (top) section had developed a leak and Employee #1 was instructed by his foreman to obtain measurements of the broken line. He gained access to the head section by climbing upon the rig's body, stepping onto the hydraulic piston assembly, and then hoisting himself to the web members of the boom (mast). In order to reach the side of the leaking hose, he stepped on two wire rope drum assemblies and continued his ascent to the hydraulic line. Once he took measurements and started down, the foreman operating the controls engaged the lever controlling the push block on the hydraulic ram. As the push block was lowering Employee #1 was descending, third, fourth, and fifth and at a horizontal web member the employee's left foot met the push block. The employee's 3rd, 4th, and 5th digits on his left foot were bruised or broken and suffered minor bleeding. One or more toes may have to be amputated.
