Employee injured in 10 ft fall

Employee #1 responded to the head saw area of a plant where a piece of equipment was malfunctioning. After controlling and locking appropriate energy sources, Employee #1 climbed the equipment's structure, rather than using a portable ladder, to access the top of the equipment and determine if a limit switch was defective. During his ascent, he fell and landed on a pile of sawdust the headsaw had created. After landing on his feet, Employee #1 lost his balance again and fell headfirst into the axle of a board unit (crane-like device that removes logs from trucks and places them onto a conveyor). He sustained minor head injuries for which he was treated at the Peninsula General Hospital in Salisbury, MD. Employee #1 was aware that portable ladders were available. He was also aware of the existing policy that ladders should be used to access all elevations.
