Employee catches foot in unguarded pinch point

The welding line that welds accessories to metal brackets at 7 different work stations was being maintenanced by Employee #1, a mechanic. When Employee #1 exited the line, he inadvertently stepped on a 22 inch by 18 inch by 7 inch guard that was guarding a pinch point. The guard was not secured and he placed his foot between a can and the floor, which was the pinch point. When the line indexed, Employee #1's foot became stuck in the pinch point. All employees wore foot protection, and the guard had provisions to secure it in place, but the screws had not been installed to secure it. All other guards were found to be secured. It could not be established who had last removed the guard and not resecured it. Employees were reinstructed and received verbal warnings. The company's rules and practices are enforced.
