Employee's head cut by falling box

Employee #1, a stock clerk, had requested an order from a hi lift operator, who was pulling the pallet from the top of a rack. Employee #1 was nearby asking customers to stand clear from the area when an approximately 40 to 50 lb unsecured box fell from the top rack and struck Employee #1 on the top of his head. He required 30 stitches.
