CONSTRUCTION EMPLOYEE STRUCK AND KILLED BY FALLING PIPES

AT APPROXIMATELY 10 AM ON DECEMBER 5, 1986, EMPLOYEE #1 WAS HELPING UNLOAD SEWER PIPE FROM A FLATBED TRAILER. THE 8 INCH DIAMETER BY 12 FEET LONG PVC SEWER PIPE WAS STACKED 5 HIGH AND 5 ACROSS, SEPARATED BY PLASTIC SPACERS. THE 25 PIPES IN EACH BUNDLE WERE HELD TOGETHER BY 1/2 INCH METAL STRAPS ON EACH END. TWO BUNDLES WERE STACKED, ONE ATOP THE OTHER, HELD TOGETHER BY ONE 1/4 INCH METAL STRAP AROUND THE MIDDLE OF BOTH BUNDLES. EMPLOYEE #1 WAS STANDING ON THE EDGE OF THE TRAILER, CUTTING THE 1 1/4 INCH STRAPS HOLDING THE TWO BUNDLES TOGETHER. THE METAL STRAPS AT THE ENDS OF THE TOP BUNDLE FAILED, CAUSING 25 SEWER PIPES, WEIGHING 125 POUNDS APIECE, TO ROLL OFF THE TRAILER AND KNOCK EMPLOYEE #1 TO THE GROUND. HE WAS COVERED BY THE SEWER PIPES AND SUFFERED FATAL INJURIES. HE DIED AT THE SCENE.
