Employee's arm fractured in unguarded point of operation

At 8:07 p.m. on September 2, 1987, Employee #1 was attempting to retrieve a card that had been missed by the second pick-up. He turned off stationary line one using a STOP switch and had reached into the point of operation when the machine apparently restarted or jogged. His right arm became caught between braces and two pick-ups on station one, and it took five minutes to free him from the machine. Employee #1 fractured his arm in three places and was hospitalized. The accident resulted from failure to guard the machine's point of operation.
