Employee killed when train strikes grader at road crossing

Employee #1, a motor grader operator, was scraping a haul road at a construction site. He drove the grader across the temporary railroad crossing that had been constructed for equipment and vehicular traffic. The grader was on the tracks when it was struck by a passing train, killing Employee #1. The necessary safety precautions were not in place at the track crossing.
