UNTRAINED EMPLOYEE FELL FROM LADDER; DIED FROM HEAD INJURY

EMPLOYEE #1 WAS FASTENING SHEET ROCK PANELS TO WALL STUDS IN A STAIRWELL WHICH LED FROM THE FIRST FLOOR TO THE BASEMENT. IT WAS 16 FEET FROM THE FIRST FLOOR LEVEL TO THE BASEMENT. THERE WAS AN INTERVENING LANDING PLATFORM 10 FEET FROM THE FIRST FLOOR. EMPLOYEE #1 USED A OPEN STEP LADDER ERECTED ON THE FIRST FLOOR AND AN ALUMINUM EXTENSION LADDER AGAINST THE FACING WALL, FROM THE LANDING PLATFORM, TO HOLD A RUNG-SUPPORTED HORIZONTAL 2 BY 10 INCH SINGLE WALKBOARD. EMPLOYEE #1 FELL FROM THE WALKBOARD AND STRUCK HIS HEAD ON THE LANDING FLATFORM, 10 FEET BELOW. THE EMPLOYEE HAD STARTED WORK THE DAY BEFORE. HE HAD BEEN WARNED ONLY MINUTES BEFORE THE ACCIDENT THAT HE WAS IN A UNSAFE POSITION, AND TO GET DOWN OFF THE WALK BOARD.
