Employee injured in fall from scaffold

At approximately 9:30 a.m. on July 24, 1991, Employee #1 apparently lost his balance and fell approximately 60 feet from the scaffold to the ground when the vertical slip form that he was lifting with coworkers collapsed. Suffering a fractured skull, Employee #1 required hospitalization.
