Employee killed when caught in pug mill

At approximately 4:40 p.m. on May 18, 1987, Employee #1 and two coworkers had just finished cleaning a pug mill. Employee #1 instructed the first coworker to start up the pug mill so he could load two more truckloads of process gravel. The second coworker walked to an area on another level by the breaker room to clean himself off. The first coworker went into the control room about 20 ft from the pug mill to start up the machine. Three switches were used in sequence to turn it on: a power switch, then a hydraulic switch, and last, a pug mill switch. The power and hydraulic switches were already in the ON position, so the coworker had only to push the pug mill switch. When he did, he heard an unusual shaking sound coming from the pug mill and immediately turned it off. Employee #1 had somehow been drawn into the pug mill and become caught between the blades. He was killed.
