Employee asphyxiated in oil when pinned by rack

Employee #1 was helping the maintenance manager to install a pin and bolt into a cylinder and quenching rack. The rack fell, pinning the Employee #1 against the tank wall and pushing his head into the quenching oil. He was crushed, and asphyxiated by the oil, and died.
