Employee dies when struck by falling cotton bales

At approximately 8:15 a.m. on March 16, 1995, in a warehouse at the Gov. Nichols Street Wharf in New Orleans, LA, Employee #1 was tagging the ends of cotton bales. She was working adjacent to 5 cotton bales which were vertically stacked. Each bale weighed approximately 505 lb. It appeared that the top 3 bales shifted, falling onto Employee #1 and inflicting fatal injuries. The stack was approximately 8 ft 6 in. high by 5 ft 6 in. wide by 4 ft 10 in. long. Because there were no witnesses, the exact cause of the bale displacement is not known. The bales had been stacked at the location at approximately 9 p.m. on March 15, 1995.
