Investigation Summary
Employees #1 through #3 were installing long span bar joists for a section of roof over a practice basketball court. Eight of the 96 ft long bar joists and one 96 ft long tie joist fell approximately 24 ft onto metal decking. This caused the three employees to also fall; two sustained serious injuries and one was treated and released. Causal factors for this accident include the employees' failure to ensure that the columns were framed in at least two directions or their installing the tie joists from column to column before beginning installation of the ball joists.
