STRUCK BY WATER UNDER PRESSURE

AN EMPLOYEE OF AN ELECTRIC UTILITY WAS ASSIGNED TO TAKE SLAG SAMPLES. THE EMPLOYEE HAD TO SHUT DOWN THE WATER SYSTEM BEFORE OPENING THE DOOR TO A 200-FOOT-TALL WATER TOWER TO TAKE THE SAMPLES. THE GATE TO SHUT OFF THE WATER SUPPLY DID NOT GET ENERGIZED. THE EMPLOYEE DID NOT CHECK THE CHAMBER TO ENSURE THAT IT WAS EMPTY. WHEN HE CRACKED THE CHAMBER DOOR, 2000 GALLONS OF WATER POURED ONTO HIM. THE EMPLOYEE DIED OF GENERAL SHOCK TO HIS ENTIRE BODY. THE CORONER'S AUTOPSY TOXICOLOGICAL SCREENING FOUND HIGH LEVELS OF PRESCRIPTION AND CONTRABAND DRUGS IN THE EMPLOYEE'S BLOOD.
