EMPLOYEE CRUSHED TO DEATH IN HOT FORMING MOLD

EMPLOYEE #1, A MAINTENANCE, WAS ADJUSTING THE STOPS ON A HOT FORMING MOLD. THE PRESS WAS IN THE OPEN MODE AND THE LOWER (MOVING) SECTION OF THE MOLD WAS BLOCKED UP BY A SINGLE 2 BY 4. THE EMPLOYEEPOSITIONED HIS BODY THROUGH THE FRAME OF THE MOLD SO THAT HE COULD ADJUST THE STOPS FOR THICKNESS. THE 2 BY 4 FELL OUT AND THE WEIGHT OF THE PRESS (1500 LBS) BROUGHT THE FRAME OF THE LOWER DIE OR MOLD DOWN. THE EMPLOYEE WAS CRUSHED TO DEATH.
