Employee killed when he falls into tank of chromic acid

At approximately 5:00 p.m. on June 8, 1991, Employee #1 was hand placing a part into a chromic acid plating tank. As he leaned over the side of the tank, which was approximately 30 in. high, his feet slipped on some rods (61 in. long by 1 1/4 in. diameter) that were lying on the floor next to the tank. Employee #1 fell headfirst into the active chromic acid plating tank #3. He died.
