Investigation Summary
Employee #1, a stone cutter for Valley Stone and Brick, stacked approximately 8,000 lb, 40 to 41 in. high blocks two high and secured each with 4 by 4 wood cribbing. The blocks tipped over onto Employee #1. The top block pinned him against a pallet of stone slabs that was left in the aisle. Employee #1 was killed. Citations were issued for violating 29 CFR 1910.176(a) and (b).
