Employee killed when crushed by gondola

Employee #1 and three coworkers were using forklift trucks to move a large gondola filled with scrapped equipment. Employee #1 stepped between one of the forklift trucks and the gondola to hook up a chain. The two lift trucks at the rear of the load pushed the gondola into Employee #1. He was crushed and killed.
