Employee killed when head crushed by overturning lift

Employee #1 was operating a Hall highlift rough terrain forklift, model 400-34, on a construction site. He was delivering materials to employees who were working on a scaffold four bucks high and approximately 25 ft high. When he backed up with the forklift's mast and boom still raised, the forklift tipped over on its side. Employee #1 was killed when his head was crushed by the vehicle's mast or lift carriage.
