Employee killed in fall from mixing tank

At approximately 7:10 a.m. on July 23, 1992, Employee #1 was pumping a batch of resin from a mixing tank into a truck. He apparently went on top of the tank, which was 15 ft high by 10 ft in diameter, to operate a valve that would refill the tank with base resin. The valve would normally be operated by a motorized actuator, but the actuator had been out of service for a year, and the valve had to be manually operated. The tank was not provided with guardrails and the employee fell 15 ft, striking the piping. He was killed.
